I understand that I am in full control of my payment, and if at any time I decide to make changes or discontinue EFT service, I can do so in person at WFOB or by certified mail only. Change of payment method will not affect other provisions and terms of my membership. *Subject to cancellation terms.

The 42-Day Get Healthy Assessment Reservation is considered a Paid In Full Agreement and provides a reservation for a spot in the upcoming Wome's Fitness of Boston 42-Day Get Healthy Challenge. Prior to the start of the 42-Day Get Healthy Challenge, I am responsible for contancting my WFOB Team Coach via email at memberservices@womensfitnessofboston.com or 617-227-1221 and scheduling and attending my initial 60-minute assement. When meeting with my TEAM WFOB COACH, I agree to review and provide billing information to complete a new agreement with terms & conditions for 6 weekly payments of $87 to be billed at the start of the 42-Day Get Healthy Challenge.  If I agree to these terms and conditions on or before 11/30/2019 I will receive the VIP Discount of $30 OFF my weekly payments during the challenge. 


Buyer represents, warrants, and acknowledges that the buyer is in good physical condition and able to utilize Women's Fitness of Boston equipment and facilities and perform the exercises available at WFOB. Buyer also agrees to use the facilities and equipment of WFOB in a safe and responsible manner, to request of WFOB when appropriate or necessary instruction on the proper use of all equipment, and the proper method of exercises. In addition, buyer agrees to warm-up properly prior to beginning any exercise program and to use the equipment only for the exercises that the piece of equipment was intended for. Buyer also agrees to use caution, common sense and safety when selection the type of machine, exercise, or amount of weight during their workout at Women's Fitness of Boston. Rules, policies, and regulation may be subject to change without notice. 

I am willingly having my picture taken and give your permission to use my picture in print, online, or for any other future media/social media and promotional purposes for Women's Fitness of Boston. I agree to be fully responsible for my own participation, and hold WFOB or representatives harmless from any liability from the use of my picture. I also consent the use of my name, picture, and any other information about me that would be beneficial to the purpose of promotion and publicity for Women's Fitness of Boston. Please note, announcements are made prior to any pictures being taken.

CANCELLATION POLICY

Failure to the schedule or attend the assessment does not relieve the participant of their financial obligation. The 42-Day Get Healthy Asessement Reservation is considered to be a paid in full agreement and this agreement can be cancelled within 3 business days of joining. Paid in Full agreements are non-refundable after 3 business days. Failure to schedule and/or attend the assessment prior to the 42-Day Get Healthy Challenge will result in a loss of reservation for 42-Day Challenge.  


Payments and Balances Due

It is understood that payments will be collected through an electronic funds transfer system using a major credit card, debit card, checking account, or savings account. All uncollected  payments due to insufficient funds, problems with accounts, or balances past due will be subject to a $10 service charge. Failure to correct these problems within 7 days of being notified will result in the $10 service charge automatically billed and charged to the billing method on file. If we do not hear from you within 30 days of being notified,  we may be forced to place the account in collection.
