START NOW FITNESS - MY FITNESS CHOICES ONLINE PERSONAL TRAINING

Here are the details of your package and the payment details:

Package Purchased:

You have purchased Each Payment MY FITNESS CHOICES which will give you unlimited access to your Each Payment 1 Online Nutrition - 1 Month of My Fitness Choices Nutrition Access, 1 Online Fitness - 1 Month of My Fitness Choices Exercise Access
You can make changes to your diet and exercise programs, track and review your progress and reset your goals as you wish.

The cost of this program is $19.99 monthly

The agreement starts April 19, 2024 with the first payment April 19, 2024 and it is True that this agreement auto-renews.

I (we) understand and agree that this agreement is classified as a perpetual commitment and my payments will continue on a month-to-month basis if I (we) do not provide written direction to START NOW FITNESS  with 30 days notice to terminate or modify payments after the agreed upon number of consecutive months have been completed.

Pre-Authorized Payment Authorization.

Terms and Conditions:

I (we) authorize START NOW FITNESS  to debit my (our) account as indicated on the credit card number supplied under the terms and conditions agreed to be me (us) with START NOW FITNESS  until such time as 30 days written notice to the contrary is given.

 

I(we) acknowledge that delivery of my (our) authorization to START NOW FITNESS  constitutes delivery by me (us) to the branch of the financial institution at which I (we) maintain an account and that such financial institution is not required to verify that the payments(s) are drawn in accordance with the authorization. Termination of this authorization done/may not terminate the contract for goods or services exchanged.

 

I (we) will notify START NOW FITNESS  in writing of any changes in the account information or termination of this authorization prior to the next due date of the pre-authorized debits. I (we) agree that START NOW FITNESS  will charge me the standard chartered bank penalty fees per rejected payment.


Cancellation Policy:
 
You may cancel this agreement at any time. You will be able to access your program until the next payment falls due. No partial refunds of monthly dues will be given. Access will terminate upon failure of your payment to process.
